If the Body Corporate Committee approves your request, this amount will be credited towards your next levy contribution period. Please note Hartley’s Body Corporate Management does not have the authority to approve the requests detailed below. Your request will be submitted to your Body Corporate Committee.

I/We acknowledge and understand that by submitting this request my Body Corporate Committee will be contacted to consider my request and that a Levy Arrears Adjustment/Payment Reversal charge in the amount of $16.50 inc GST and/or Penalty Waive Request charge in the amount of $22.00 inc GST will be applied to my Lot.

I/We acknowledge and understand that the following charges are levy recovery costs and if the Body Corporate Committee approves for these costs to be waived from my Lot my Body Corporate (made up of all unit owners) will still be liable to pay these costs as noted below (if applicable):

• Charge for arrears notice—$44.00 inc GST
• Charge for final notice—$66.00 inc GST
• Final Notice Phone Call—$16.50 inc GST
• Letter of Demand—$176.00 inc GST

I/We acknowledge and understand that if my Committee approves my Payment Plan, a Payment Plan Management charge of $66.00 inc GST will be applied to my Lot for Hartley’s Body Corporate Management to monitor my repayments during the course of my payment plan, and to notify the Committee if I default on my repayments.

All charges above-mentioned in the Conditions are in accordance with the added fees outlined in the Management Agreement with Hartley’s Body Corporate Management.

I/we acknowledge and understand that for the duration a debt is owed to the Body Corporate I/we will be ineligible to vote at General Meetings and / or nominate for a position on the Committee.
